Case Manager

Overview

The Case Manager is responsible for assessing client needs, developing comprehensive care plans, and ensuring access to essential services. They play a key role in advocating for clients and monitoring their progress toward stability and success.

 

Responsibilities

  • Client Assessments: Evaluate individual needs and create personalized service plans.
  • Resource Coordination: Connect clients with healthcare, housing, education, or employment assistance.
  • Monitoring & Follow-Up: Track progress and adjust service plans as needed.
  • Crisis Intervention: Provide support during emergencies or difficult situations.
  • Documentation & Compliance: Maintain case records and ensure compliance with regulations.
  • Collaboration & Referral Management: Work with social workers, healthcare providers, and agencies to support clients.
 

Qualifications

  • Education: A bachelor's degree in social work, psychology, human services, or a related field is typically required. A master's degree in social work (MSW) or counseling is preferred for higher-level roles.
  • Experience: 2-5 years of experience in case management, social services, or counseling.
  • Technical Skills: Familiarity with case management software, documentation tools, and client tracking systems.
  • Communication & Interpersonal Skills: Strong ability to connect with clients and collaborate with service providers.
  • Problem-Solving & Advocacy: Ability to assess client needs and advocate for necessary resources.
  • Cultural Competency: Understanding diverse populations and addressing barriers to care.
